Courses: Practice based pharmacist development programme: Practice activities
The main focus of the PBP development programme is on in-practice training and experiential learning in the workplace. This provides pharmacists with the opportunity to gain knowledge, skills and experience in a range of practice areas core to the role of a practice-based pharmacist, namely:
Understanding and working within the NHS
Clinical assessment and management
As part of this structured development programme, pharmacists will undertake practice activities to help them to develop, demonstrate and evidence their competence in each practice area. See below for practice area-specific forms and information. Pharmacists should record CPD cycles using the PSNI’s online system and print and scan the document to upload to their portfolio.

Each care plan to include medication review or medicines reconciliation and recommendations for prescribing/de-prescribing. One care plan to be recorded for each of these four clinical areas – CV, respiratory, diabetes and pain
