Aim
To introduce pharmacists to the basic skills required in the provision of a patient-centred clinical pharmacy service.

Intended learning outcomes
Having completed this workshop you should be able to:

develop a systematic approach to identifying medication-related problems and making appropriate recommendations
construct pharmaceutical care plans (PCPs) for individual patients
reflect on the delivery of holistic patient-centred pharmaceutical care to those patients.

NB the number of hours awarded includes pre-work in addition to the webinar session itself
