Aim
To equip pharmacists to support patients living with long-term conditions to self manage their conditions and medicines effectively and to take ownership of their health.

Intended learning outcomes
Having completed this workshop you should be able to:

• Undertake a comprehensive medication review
• Promote the benefits of self management of long-term conditions to patients
• Assess adherence to prescribed medication
• Encourage health ownership
• Understand the importance of medicines reconciliation in supporting those recently discharged from hospital
• Signpost patients and their careers to appropriate support services.

Additional information
This two-night regional course is 'open access' which means that pharmacists and pharmacy technicians need not apply to attend but simply turn up at the venue on the given date. PLEASE NOTE THE START TIME OF 7:30PM.
