Aim
To support pharmacists in undertaking medicines reconciliation and medicines review within the care home setting whilst adhering to NICE good practice guidance for managing medicines in care homes.

Intended learning outcomes
Having completed this workshop you will be able to:

• appreciate the importance of implementing a care home medicines policy
• outline the process of medicines reconciliation and who should be involved in this process
• undertake medicines reconciliation and medicines review for residents in a care home setting
• ensure resident records are accurate and up-to-date
• understand the processes for prescribing and issuing prescriptions for residents of care homes
• outline the process of self-administration to empower residents to take their medicines independently.
