Aim
To enable pharmacists to develop a systematic approach to carrying out a medication review which will identity and resolve medication-related problems.

Intended learning outcomes
Having completed this workshop you should be able to:

• compile an accurate drug history
• identify, access and evaluate relevant sources of information
• identify medication-related problems with reference to patient safety and clinical and cost-effectiveness
• determine the reasons why such problems occur
• make recommendations to resolve medication-related problems
• understand the importance of medicines reconciliation.
